ABSTRACT

Sepsis, a common critical disease in the intensive care unit(ICU), features high morbidity and mortality. At present, it is mainly tackled with western medicine, which may trigger a series of problems like antibiotic resistance, adverse hormonal reactions, and high cost after a long-term use. Therefore, exploring new efficient, safe, and cheap drugs and treatment modes has become the focus of our research at this stage. By virtue of unique advantages including "the concept of holism and individualized treatment based on syndrome differentiation", Chinese medicine has accumulated quite rich experience in the prevention and treatment of sepsis. In recent years, research on the regulation of Chinese medicine on nuclear transcription factor-κB(NF-κB) signaling pathway in sepsis has kept emerging. On this basis, this paper reviewed the etiology and pathogenesis of sepsis, syndrome differentiation and treatment, NF-κB signaling pathway, and its intervention with Chinese medicine. It has been found that some single Chinese herbs and their extracts, Chinese herbal compounds, and Chinese herbal injections effectively inhibit the expression of such inflammatory factors as NF-κB-mediated tumor necrosis factor-α(TNF-α), interleukin-1(IL-1), and IL-6 as well as the related proteins, reduce the systemic inflammatory response and organ injury, and improve the prognosis by regulating the activation of NF-κB signaling pathway and the immune function of macrophages. However, due to the limitations of objective conditions, some studies also have the problems of fuzzy pro-inflammatory anti-inflammatory balance mechanism, unclear pharmacokinetics and low drug safety evaluation, which need to be further studied and explored in order to provide a new theoretical basis and diagnosis and treatment thinking for the treatment of sepsis with traditional Chinese medicine.

ABSTRACT

BACKGROUND: The tooth ash can be used as a scaffold for bone tissue growth and provide calcium and phosphorus components during bone regeneration. Platelet-rich plasma can promote the soft and hard tissue regeneration. However, either of them has its shortcomings.OBJECTIVE: To investigate the effect of platelet-rich plasma combined with tooth ash in repairing skull bone defects. METHODS: Nine healthy rabbits were selected to make bone defects on both sides of the skull. Rabbit platelet-rich plasma combined with tooth ash was implanted into the skull defect on the left side as experimental group, while rabbit platelet-rich plasma was implanted into the skull defect on the right side as control group. Skull samples were taken out at 4, 6, 8 weeks after implantation for soft X-ray detection, hematoxylin-eosin staining and modified Gomori staining. RESULTS AND CONCLUSION: (1) Soft X-ray: The trabecular bone area of the experimental group was larger than that of the control group at 6 and 8 weeks after implantation (P < 0.05 or P < 0.01). (2) Hematoxylin-eosin staining: With the duration of implantation, newly formed fibers with bone structure gradually reduced at the defect sites in both groups, and there was a orderly layered arrangement in the bone structure and increased calcification. Compared with the control group, relatively higher new bone maturity, better bone trabecular arrangement and more osteoblasts were observed in the experimental group. (3) Modified Gomori staining: With the prolongation of implantation time, the new bone became mature gradually in the two groups, and the bone maturity in the experimental group was higher than that in the control group. To conclude, platelet-rich fibrin combined with tooth ash is better to promote bone defect repair.

ABSTRACT

<p><b>OBJECTIVE</b>To study the magnetic resonance imaging (MRI) in the wrist joint of coal miners who work in excavation and vibration department.</p><p><b>METHODS</b>Forty-three coal miners with the hand-arm vibration disease served as the observation group while 20 workers who were not working in the vibration department acted as the control group. The patients in the observation group were divided into five subgroups according to the time when they received vibration. The regularity of the development of signs and symptoms of MRI was observed and analyzed.</p><p><b>RESULTS</b>The hydroarthrosis was most found in MRI. There were significant difference in hydroarthrosis (chi(2) = 8.80, P < 0.01), osteoporosis and osteomyelitis (chi(2) = 3.91, chi(2) = 5.01, P < 0.05 respectively) between the observation group and the control group. The edema of bone marrow and the avascular necrosis of ossa carpi were found only in the observation group and not found in the control group. The hydroarthrosis and the edema of bone marrow occurred most in the early stage of vibration. The signal in the edema of the bone marrow of the distal end of the radius was decreased in the GE sequence T(2)WI with the specificity.</p><p><b>CONCLUSION</b>(1) Changes in the wrist joint occur in the early stage of the vibration work, and can be found in the MRI. (2) The edema of the bone marrow of the distal end of the radius is of great value in the diagnosis of the hand-arm vibration disease.</p>
